Join us at ECCMID 2020 to experience the Symposium “Understanding and shaping the respiratory tract microbiome” co-organized by ESGB (ESCMID Study Group for Biofilms) and ESGS (ESCMID Study Group for Staphylococci and Staphylococcal Diseases)
Speakers: Debby Bogaert, Silvio Daniel Brugger, Sarah Lebeer, Michael Tunney The aim of this symposium is to provide scientific background on the development of the upper respiratory microbiome after birth and during later stages of life. The goal is that participants obtain the latest knowledge on microbiota dynamics and influencing factors in association with health and disease. Furthermore, speakers will present insights into cutting-edge molecular mechanistic studies investigating the interplay between the host and its microbiota as well as microbe-microbe interactions, with the goal to illustrate future preventive and therapeutic options.

CMI Highlight: Infections caused by extended-spectrum beta-lactamase-producing Enterobacterales after rectal colonisation with ESBL-producing Escherichia coli or Klebsiella pneumoniae
Infections due to extended-spectrum beta-lactamase-producing Enterobacterales (ESBL-E) are considered infections with a high public health burden. In this study, L. Denkel et al. aimed to identify incidences of and risk factors for healthcare-associated infections (HAI) after rectal colonisation with ESBL-producing Escherichia coli (ESBL-EC) or Klebsiella pneumoniae (ESBL-KP). Patients colonised with ESBL-EC or ESBL-KP were monitored for subsequent HAI with ESBL-E, and other pathogens.
The authors found that the risk of extra-intestinal infection with ESBL-E from the gut was increased in patients colonised with ESBL-KP but not in patients colonised with ESBL-EC. Patients with ESBL-EC were more likely to acquire a HAI with a different bacterium than with rectally colonising ESBL-EC. Patients rectally colonised with ESBL-KP have an increased risk of subsequent infection with their rectal ESBL-KP. Clinicians should be aware of this species-specific difference that might be caused by underlying diseases, higher pathogenicity of ESBL-KP compared to ESBL-EC and other factors that are not understood, yet.
