CMI highlight: Antibiotics: Easier to start than to stop? Predictors of antimicrobial stewardship Recommendation acceptance

Acceptance of prospective audit and feedback antimicrobial stewardship program (ASP) recommendations has been shown to vary, but the drivers of recommendation acceptance are not well understood. The authors here sought to identify the factors associated with recommendation acceptance at a large community teaching hospital. 

Data from an ASP recommendation registry were collected from 2010 to 2018. Variables included data about the infection, prescriber, and recommendation, categorized by whether it increases, decreases, or is neutral to antibiotic exposure. The primary outcome was acceptance of ASP recommendations. Adjusted odds ratios and 95% confidence intervals were estimated using logistic regression models with random intercepts in order to account for clustering by prescriber. 

The present cohort study of over 11,000 ASP recommendations identified key predictors of recommendation acceptance, most notably that recommendations that aim to reduce antimicrobial exposure have significantly lower odds of acceptance compared to recommendations that increase exposure for patients. This contrasts with the aim of reducing unnecessary antimicrobial use thereby decreasing selective pressure and subsequent antimicrobial resistance.
